OUR HISTORY

Grace Health & Rehab Center has been caring for seniors in our community since 1980 – earning a reputation for the highest quality nursing care. Throughout more than 27 years of operation, the facility has built a proud history of providing long-term care for the residents of East and West Feliciana, East Baton Rouge, St. Helena and Tangipahoa parishes, among others. The facility is located 15 miles north of Zachary and two miles south of Clinton, Louisiana – just off Highway 67 (Plank Road). Grace Health & Rehab Center is a professionally managed long-term care facility that is staffed by members of the local community. The facility is Medicare/Medicaid certified and is a member of the Chamber of Commerce, Louisiana Nursing Home Association and American Healthcare Association.
